Penalty for the One Care Street Health Risk
 Assessment Survey: You Can Make it End.

March 19, 2009 is the next pay day and should be the last for your deduction for failure to complete the Risk Assessment Survey.  If you (or your spouse) have not completed the survey, it MUST be done IMMEDIATELY to avoid further monthly deductions.   If you have already complied by completing the Survey, please remind your fellow members (or their spouse) to complete the survey to avoid further deductions to their pay check. If you receive a One Care Street Health Coach call you are obligated to listen to the advice to be in compliance.  You must take the call to be in compliance. 1(866)245-2453 is the One Care Street phone number if you need assistance.
