One Care Street Health Survey 

Dec 3, 2008 Update: AT THIS TIME ALL CSEA UNION MEMBERS USING POMCO FOR HEALTH INSURANCE SHOULD COMPLETE THE ONE CARE STREET HEALTH SURVEY AND PRINT THE LAST "COMPLETION" PAGE FOR YOUR RECORDS BEFORE DECEMBER 15, 2008. Failure to do so will require payment of 20% of the cost of the POMCO plan by you for 3 months until the survey is completed. 1(866)245-2453 is the One Care Street phone number if you need assistance. The survey may be completed during work hours at a County computer or from home. The Client Number for our survey is #187.     www.onecarestreet.com  is the starting web page. If the survey is completed by you (and your spouse/partner) by December 15, 2008 you will indeed be in compliance.  If One Care Street schedules a "planning call" to you dated after December 15 you will still be in compliance. If you get a call you are only obligated to listen to the advice that is given to be in compliance.
